Potter County Human Services

Coordinated Services is a unit within the Human Services system which provides for the administration and service delivery of the system’s miscellaneous programs and grants. Specifically, Coordinated Services offers case management and service planning activities for Potter County residents who meet eligibility requirements and require such services.

The purpose of Coordinated Services is to:

  • Provide services to clients ages 18 through 60  with various needs 
  • Serve as a resource to the Human Services system
  • Appropriate new services; and
  • Expand existing resources as additional needs are identified by the system

Funding for Coordinated Services is through various grants and programs. Specifically, the grants include Human Services Development Fund (HSDF), Medical Assistance Transportation Program (MATP), Emergency Food and Shelter programs (EFSP), State Food Grants (TEFAP), and funds from some of the categorical programs in the agency.

Car Seat Program

Child car seats are rented to eligible individuals who will then pick up, sign for, and pay one dollar per month. These car seats meet all safety requirements.

Emergency Food & Shelter Programs

Emergency Food and Shelter Programs (EFSP) are designed to provide assistance for needy individuals in Potter County if they are homeless or potentially homeless. When applying for the grant, individuals must provide:

  • Verification of need
  • Family information
  • Financial status
  • Other available resources

The service provides temporary shelter to a family in need, or months’ rent. Case management is offered to assist the client in obtaining adequate housing by working closely with the housing programs and individual landlords in the area. Case management assistance is also offered for ongoing needs. Referrals to other agencies are made when appropriate.

Food Banks

The Food Grant programs are funded through the Emergency Food and Shelter Program and through the USDA food programs. The five area food banks receive food from these programs as administered by this agency. The food programs are designed to assist eligible clients to receive food either monthly because of chronic need, or on an emergency basis.

Limited funds are also available to provide one-time food vouchers for qualifying persons. Here is a list of your community food banks.

Homeless Assistance

The Homeless Assistance Program is funded to assist individuals that are homeless or in danger of becoming homeless. Following the initial intake, the HAP caseworker works with the client to assist in obtaining:

  • Adequate housing
  • Provision of rental assistance
  • Linkage of participants with appropriate programs (Social Services, Housing Authority)
  • Development of a service plan (which documents goals that client anticipates successfully achieving during their participation in the program)
  • Completion of an exit interview when client finishes the program
Information & Referral

Information and Referral services are available to the general public. Services are provided to walk- ins or by telephone, and now through the Internet. Information is available on a wide range of subjects. Pamphlets, tax rebate and other applications are available. Referrals to other agencies are made when appropriate.

Medical Assistance Transportation(MATP)

The Medical Assistance Transportation Program (MATP) is funded by the State Department of Public Welfare. It is designed to assist persons who are eligible Medical Assistance recipients to receive funds to offset the transportation costs involved in obtaining medical treatment and prescription drugs.

Caseworker assistance includes:

  • Initial interview and application with the client to determine eligibility for the program
  • Maintaining records of the number of trips taken;
  • Collection of vouchers and determination of reimbursement amount; and
  • The issuance of checks to clients

Rental Assistance
